The South East Toronto Family Health Team (SETFHT) is a group of family physicians, medical residents and health care professionals who provide primary care services to approximately 12,000 patients in the East York area. SETFHT is associated with the University of Toronto Department of Family and Community Medicine and is funded by the Ministry of Health and Long-Term Care. Helping You Stay Healthy A Family Health Team is a new way of providing primary health care in Ontario. It is a team approach that brings together family physicians with other health care providers in order to coordinate the best possible care for patients.
