What We Do

The South East Toronto Family Health Team (SETFHT) is a group of family physicians, medical residents and health care professionals who provide primary care services to approximately 12,000 patients in the East York area. SETFHT is associated with the University of Toronto Department of Family and Community Medicine and is funded by the Ministry of Health and Long-Term Care.

Helping You Stay Healthy

A Family Health Team is a new way of providing primary health care in Ontario. It is a team approach that brings together family physicians with other health care providers in order to coordinate the best possible care for patients. Everyone on the team works together to meet your needs. Our focus is on keeping you and your family healthy, not just treating you when you are sick.

When you enroll as a patient with our Family Health Team, you are taking a step to ensure that you always have convenient access to regular check-ups and ongoing health care from a family physician and/or a team of other health care providers.

Your primary care team at SETFHT consists of many team members:

• Mental Health Addictions Counsellor
• Social Worker
• Care Navigator

Each member plays a different role in your health care needs and you and your family will benefit from a comprehensive approach to health care.
