Heart & Breathing
- Breathing problems
- Chest pains or racing heartbeat
- Difficulty speaking or swallowing
Neurological Symptoms
- Numbness or weakness of the face or body
- Loss or change of vision
- New confusion or memory problems
- New or worsening seizures
Severe Illness
- Neck stiffness or severe headache
- Vomiting and inability to drink fluids
- Fever/severe pain that is not improved with acetaminophen/ibuprofen
- Pregnancy and labour problems
Injuries & Mobility Issues
- Serious injuries (e.g., bleeding that won’t stop, deep cuts, head injury)
- Possible major broken bones (e.g., bones or joints look different, cannot put weight on injury)
- Inability to walk

- Narcotic Prescriptions
- Completion of Forms
- Immunizations
- Prescription renewals
