South East Toronto Family Health Team is committed to providing patient centered care in our community.  One way we are doing that is through our patient advisory council (PAC).

What is a PAC?

It is a committee made up of SETFHT patients who volunteer their valuable time to provide feedback to us to make us a better leading academic family health team.

How do we do this?

 We meet a minimum four times a year to hear your ideas, opinions, and experiences, as well as to get feedback on new FHT initiatives. We need your input!

From time to time the PAC will be accepting new members
