Accessibility at SETFHT
The South East Toronto Family Health Team (SETFHT) is committed to providing accessible and inclusive services to all individuals. We strive to ensure that our facilities, programs and services are accessible to everyone in accordance to the Accessibility for Ontarians with Disabilities Act (AODA), the Human Rights Code and the Integrated Accessibility Standards Regulation (IASR). The purpose of the Accessibility Policy is to ensure that SETFHT is compliant with the Government of Ontario’s legislation and the Customer Service Standard. In order to remain in compliance, this policy shall be reviewed annually, in response to business needs as well as to any changes in legislation.
Feedback regarding the way SETFHT provides services to people with disabilities or to request information in a format that takes a person’s disability into account can be made by:
- Filling out our online Patient Feedback Form
- Speaking directly with a Pod Lead or any member of the clerical staff*
- Emailing our HR department (humanresources@setfht.on.ca)
- Calling 416-423-8800 ext.402 and leaving a message
*If you require a physical version of the feedback form, a paper copy is available upon request at any reception station at all SETFHT sites.
