At the South East Toronto Family Health Team, our Social Workers provide short-term mental health services to individuals 18+. We offer both individual therapy and group programs.
Individual therapy:
Our social workers offer short term (typically 6 – 8 sessions) individual therapy for the following concerns:

- Situational stressors (i.e. work/major life changes)
- Interpersonal stressors (We do not offer couples and/or family therapy)
- Grief (bereavement)
- Anxiety
- Depression
- Emotional regulation (i.e. anger, hypersensitivity)
- Postpartum adjustment, anxiety, and depression
- Sleep concerns
- Adjustment concerns
Who is Eligible?
Any patient who would benefit from a short-term therapy model, who doesn’t have access to sufficient coverage for psychotherapy through their insurance benefits from work, spouse, parents, family, or does not have access to EAP (Employment Assistance Program) therapy sessions. We will ask for benefits coverage/EAP details upon intake so please have this information available to assist with the eligibility assessment.
Referral:
We’re here to help you! Speak to your physician or nurse practitioner at SETFHT to ask for a referral.
We will complete a short telephone intake to assess eligibility and assist with the next steps. If eligible, you will be booked with one of us for short-term therapy, and if not eligible, we will help to get you connected to the appropriate resources.
