Home Visit Program
It is not easy for some patients to see their family doctor. They might have trouble walking. They might not be able to take the bus or drive. They might be too sick to leave the house.
If you are a South East Toronto Family Health Team (SETFHT) patient and you cannot leave your house, we will come to you!
You are part of the team and we will work together to plan your care and answer your questions about your health. We can also help you with other issues that might affect your health and well-being, such as meals and housing.
We want to work with you to keep you as healthy as possible.
If you are not already a SETFHT patient and want to join our Home Visit Program, ask your doctor or Home Care Coordinator to complete the Home Visit Referral Form and fax the completed form to the attention of Vanessa De la Vega, Programs & Services Assistant, at 416-423-8803.
