Hypertension Management Program
SETFHT’s Hypertension Management Program is run in participation with the Ontario Stroke Network and the Ontario Heart & Stroke Foundation. The program helps patients to lower their blood pressure by teaching them to set a series of small, reasonable lifestyle goals and become actively involved in managing their own care. When patients come in for their blood pressure check, our Registered Nurses will provide counselling, resources and support to help patients achieve their goals. Our Registered Dietitians can support patients by teaching them how to make small but meaningful changes to their food choices. And several members of our team are trained to provide smoking cessation counselling to patients who would like to use Nicotine Replacement Therapy to help quit.
This program is open to patients who 18 years and older with primary hypertension or elevated blood pressure on two or more office visits.
