INR Clinic
Patients on warfarin at South East Toronto Family Health Team can choose to have their INR monitored at the warfarin clinic instead of attending traditional laboratory monitoring. This is a pharmacist-managed clinic, held on Monday (1871 Danforth ) from 8:30 a.m. to 12:00 p.m. & Tuesday (840 Coxwell ) from 8:30 a.m. to 12:00 p.m. for our ~130 patients on warfarin.
Pharmacists & other healthcare providers, including medical residents, use point-of-care technology (Coaguchek®) to measure INRs and this allows for immediate changes to patients’ warfarin doses for better results and increased patient safety.
This clinic has been operating since April 2008.
