NOTICE TO PATIENTS AND COMMUNITY PARTNERS

Accreditation Survey – May 12 to May 14, 2025

Dear Patients and Valued Community Partners,

We are happy to let you know that the South East Toronto Family Health Team (SETFHT) will undergo an Accreditation Survey conducted by Accreditation Canada from Monday, May 12 to Wednesday, May 14, 2025, at our 840 Coxwell Avenue and 1871 Danforth Avenue locations.

As part of this process, surveyors from Accreditation Canada will be on-site to evaluate our services, programs, and care practices. This assessment reflects our commitment to delivering safe, high-quality, patient-centered care. Accreditation offers us an important opportunity to reflect on our strengths and identify areas for improvement so that we can better serve you and our community.

All services will continue as usual. However, surveyors may observe patient flow and appointments during the survey period. Patients and visitors may also be invited to share feedback about their experience with SETFHT.
