Standards for Home Care

SETFHT’s own lead physician, Dr. Tia Pham, has been included in an Expert Advisory Group which aims to develop a framework for national principle-based standards for home care.  Along with the Canadian Home Care Association (CHCA) and a panel of fellow recognized home care leaders, Tia will be guiding the development of these standards to ensure consistent and equitable access to high quality home care services.  Wonderful work Tia!  Read more about the initiative here: http://www.newswire.ca/news-releases/work-begins-on-national-principle-based-home-care-standards-630088713.html

PARKING AT 840 COXWELL AVE

Dear Patients,

Due to the ongoing construction at Michael Garron Hospital (MGH) , parking has become very frustrating for our patients. We encourage all patients to give yourself extra time when traveling to appointments. We understand this is very inconvenient and has also been a disruption for our entire team. The construction plan at MGH is 4-7years, so we do not see this being resolved anytime soon.  We strongly recommend either taking TTC, taxi or have a friend or family member drop you to appointments.

Public Transit:

  • We are located just north of the Coxwell subway station.
  • The Coxwell subway station is located at 1568 Danforth Ave., between Greenwood and Woodbine along the Bloor-Danforth line.
  • At Coxwell subway station take the northbound bus (#70 or #70A) to Sammon Ave or Mortimer Ave – Buses are about every 9 minutes.

Sammon Avenue Visitor Lot 

7am – 6pm  - $3.00 20 minutes (max $15.00)

   

6pm – 7am $7.00 Flat Rate

Call for Patient Advisory Council Members

South East Toronto Family Health Team is committed to providing patient centered care in our community.  One way we are doing that is through our patient advisory committee.

What is a PAC? It is a committee made up of SETFHT patients who volunteer their valuable time to provide feedback to us to make us a better leading academic family health team.

How do we do this?  We meet as a committee approximately four times a year to hear your ideas, opinions, and experiences, we need your input!

Where we the Committee meet? Our advisory committee will meet at our 840 Coxwell Ave. Clinic location with a facilitator at a time that is convenient for ALL members.

This is an exciting opportunity for your voice to be heard and assist in making SETFHT a better team in order to improve the health of our community.

After hours clinic is by appointment only.  Please call us at 416-469-6464 to schedule an appointment with the Staff Physician on call.

After hours clinics are for urgent medical care only.

Patients with any of the following routine needs should schedule an appointment during normal clinic hours:

  • Narcotic Prescriptions
  • Completion of Forms
  • Immunizations
  • Prescription renewals
